Summary

This Scottish Government funded programme was set up to support development of co-produced, person centred, self management activity across Scotland. Learning from people’s lived experience of living with long term conditions, and their unpaid carers, is at the heart of what the Fund is designed to achieve.

In 2024 the Self Management Fund aims to support projects that put people and communities at the heart of prevention and self management. Applications must demonstrate their project supports, guides and enables self management confidence in at least one of the following ways:

  • Supporting people living with, or who might be at risk of developing, long term conditions to be better able to self manage and/or reduce risk factors.
  • Supporting people who experience barriers to accessing health and social care support and services, due to geographical, social, or economic factors to feel better able to self manage through direct support or support to access services.
  • Supporting people who are waiting for a specialist health or social care intervention or rehabilitation support through enabling them to develop self management capabilities while awaiting treatment.
  • Supporting disabled young people or young people living with long term conditions or unpaid carers to understand the importance of self management and to develop skills which maximise their health and wellbeing.

Who can apply

Constituted not-for-profit organisations, such as charities, CICs, and community groups with;

  • A minimum of three Directors/Trustees/Board Members, the majority of whom receive no payment from the organisation.
  • A bank account in the name of the organisation that requires dual authorisation for transactions.
  • Appropriate safeguarding policies and procedures.
  • A governing document which contains: A charitable dissolution lock, charitable objectives and powers aligned with the activity proposed, and stipulations ensuring that no more than 49% of board members (including trustees/directors) can be paid for services either in their capacity as a board member or as a staff member.
